This issue covers Amazing Fantasy 15 and Amazing Spider-Man (1st series) 1 thru 29 and Annuals 1 and 2. Each issue reviewed in the index has a one-page summary and a reduced-size reprint of the original cover. Index contributors and consultants include George Olshevsky, Mark Gruenwald, Mark Evanier, Dennis Mallonee, Lou Mougin, Murray Ward, and Peter Sanderson. Cover by John Byrne. The cover portrays the Amazing Spider-Man as he was first drawn by Steve Ditko in the silver age. The back cover by Steve Ditko is the original version of the cover to Amazing Fantasy 15. The splash page features an essay on the background of the Amazing Fantasy title and reduced-size versions of the splash pages from Amazing Fantasy 15 and Amazing Spider-Man 1 respectively. The back inside cover describes the plan and purpose for the Marvel Index series. (Notes: There are nine issues in the Amazing Spider-Man Index, which was the first index series published by Marvel. The index issues do not contain any ads.)
